- Manufacturer’s Statement of Origin or Certificate of Origin, assigned to buyer
- Kentucky Application for Certificate of Title/VTR (TC96-182)
- Commonwealth of Kentucky insurance policy including the following:

- Name of the insured
- Vehicle Identification Number (VIN)
- Effective Date (current)
- a current effective date if coverage already exists, or
- a policy period that includes today’s date
- NAIC code
- The insurer’s NAIC (National Association of Insurance Commissioners) company code — a 5-digit number identifying the insurance company.

- Kentucky ID & Social Security number
- Dealer invoice (for road usage tax valuation)
- Sheriff inspection for vehicles purchased out of state
Important Notes:
- All new vehicles must be titled and registered before operation.
- Any liens must be noted and documented.
- Valuation for tax may be based on dealer invoice/MSRP.
- Vehicles purchased from out of state will need a Sheriff inspection
Fees:
- $9 title fee
- 6% road usage tax based on sale price/invoice
- $21 registration/plate fee (base amount; fees may vary for specialized plates and vehicles over 10,000 lbs)
- $22 lien filing fee, if applicable
