Documents Required:

  • Completed Application for Disabled License Plate or Parking Placard (TC 96-347)
  • Medical certification signature on the form by an authorized provider:
    • Licensed Physician, Physician Assistant, Chiropractor, or Advanced Practice
      Registered Nurse
  • Applicant’s signature notarized — County Clerks can notarize at any office.
  • Valid Kentucky ID

Important Notes:

  • If you are unable to physically make it into the office please use our drive-thru service at our branch location or call the motor vehicle department at(502) 863-7875 and if possible, we will send a deputy clerk out to your vehicle to notarize your signature for the process
  • Permanent placards are valid for 6 years and may be renewed indefinitely with a new application signed by the applicant for each renewal
  • Temporary placards are valid for 3 months and require a new medical certification signature to renew
  • If a placard becomes damaged, lost, or stolen, we will issue a replacement for a fee
  • All placards must be displayed on the rearview mirror or dashboard if no mirror is present an removed when driving

Fees:

  • Permanent Placard (first issuance): Free
  • Second or replacement placard: $10
  • Temporary placard: Free (replacement fee $10)
